Statement of Explicit Consent within the Scope of the Law on the Protection of Personal Data

Within the framework of the General Disclosure Text regarding the Law on the Protection of Personal Data presented to you by ISSCA in accordance with the relevant provisions of the Law on the Protection of Personal Data No. 6698 (“KVK Law”), I hereby acknowledge and declare that:

My personal data may be processed by ISSCA, in its capacity as data controller, or by data processors authorized by ISSCA, provided that necessary security measures are taken, for purposes including, but not limited to, providing ISSCA’s services under the best possible conditions, ensuring the secure and uninterrupted provision of these services, maximizing patient satisfaction, processing payments, carrying out various operations related to these services, managing and improving operations, promoting, marketing, advertising, and conducting campaigns for these or other products and services, informing customers about opportunities, campaigns, and other services, and fulfilling the requirements of agreements entered into with customers. Such data may include, but is not limited to, identification details, address details, contact information, and other personal data. These may be processed, obtained, recorded, stored securely either physically or electronically for the appropriate duration in accordance with the law, altered, reorganized, disclosed and transferred in accordance with legal regulations, inherited, classified, and processed or restricted from use, all in accordance with the general principles stated in Article 4 of the Law on the Protection of Personal Data No. 6698 (“KVK Law”), including the principle of retention for the period prescribed by law or for as long as necessary for the purpose of processing.

I acknowledge that I have been informed by ISSCA regarding the above matters and that I consent, within the framework of the KVK Law, to the processing of my personal data as described above.

Furthermore, I accept and declare that, for the purposes specified above, ISSCA may transfer my personal data to ISSCA employees, authorized personnel, legally authorized public institutions and organizations, other natural and legal persons in order to carry out its activities, independent auditing companies within the framework of legal obligations and restrictions, survey companies, and business partners and service providers from whom ISSCA receives services or collaborates to carry out the services and/or activities provided to me. I hereby give my explicit consent in this regard.

In addition, pursuant to Article 11 of the KVK Law and the relevant legislation, I acknowledge that I have the right to apply to ISSCA to: learn whether my personal data is being processed, request information if my personal data has been processed, learn the purpose of processing and whether they are being used in accordance with this purpose, know the third parties to whom my personal data has been transferred, either domestically or abroad, request the correction of any incomplete or incorrect personal data, request the deletion or destruction of my personal data in the event that the reasons requiring its processing cease to exist, request that third parties to whom my personal data has been transferred be notified of any corrections or deletions, object to any negative outcome that may arise as a result of the exclusive processing of my data by automated systems, request compensation for damages incurred as a result of unlawful processing of my personal data.
